Added by the utility. Community water fluoridation is a deliberate public health measure. The U.S. Public Health Service recommends a single optimal concentration of 0.7 mg/L, a level set in 2015 to balance cavity prevention against the risk of dental fluorosis, the cosmetic mottling of tooth enamel that can occur with excess fluoride exposure during early childhood. CDC data shows community water systems have generally hit that target closely, with more than 99.99% of monitored systems staying under the separate 2.0 mg/L secondary standard between 2016 and 2021.
Naturally occurring. Fluoride also leaches into groundwater from rock and soil, independent of any utility decision. Levels in untreated groundwater can range from a trace to more than 10 mg/L depending on local geology, and bedrock wells carry more of that risk. Private wells are not covered by the Safe Drinking Water Act the same way public systems are, so a well owner is responsible for testing and deciding on treatment. The 4.0 mg/L enforceable limit is far above the 0.7 mg/L fluoridation target. A result between 0.7 and 2.0 mg/L is within the range utilities aim for or tolerate; it is not the same as an exceedance of the enforceable health limit.

Fluoride policy has moved faster than most contaminant topics on this site, and it is worth separating the science from the state-level lawmaking.
In August 2024, the National Toxicology Program published a systematic review concluding, with moderate confidence, that fluoride exposure above 1.5 mg/L, roughly twice the U.S. fluoridation target, is associated with lower IQ in children. That finding was drawn mostly from studies conducted outside the United States, where exposure levels were often higher than typical U.S. community water fluoridation. The following month, a federal court in California ruled in Food & Water Watch v. EPA that fluoride at the levels typically found in U.S. drinking water poses an unreasonable risk under the Toxic Substances Control Act, and ordered EPA to take regulatory action. That ruling is being appealed.
Separately, state legislatures have started acting on their own. Utah's HB 81 made it the first state to ban community water fluoridation statewide, effective May 7, 2025. Florida followed with SB 700, restricting local governments from adding fluoride to public water, effective July 1, 2025. Other states have debated similar bills.
CDC and the American Dental Association have not changed their position that fluoridation at the recommended 0.7 mg/L level is safe and effective for reducing cavities. The court ruling and state bans reflect a live legal and legislative dispute, not a new EPA rule, an ADA reversal, or new evidence that your specific utility's water has crossed the enforceable 4.0 mg/L health limit.
